Professional development hours can be earned for conference attendance. Self-verification forms are provided to all attendees upon check-in onsite at the CASQA Registration Desk.

Instructions

  • For ECI Certification offered by CWEA: Completed forms may be scanned and emailed to CASQA@casqa.org. Certificates will be emailed to you following review and approval.
  • For All Other Crediting Organizations: Complete and keep the form for your records. Go to the crediting organization’s website for further instructions.
